Main duties of the job
- Complete timely clinical triagefor all referrals, including urgent pathways.
- Use clinical expertise to make informed triage decisions and support SPA/Care Coordination processes.
- Support and occasionally lead MDT triagefor complex cases.
- Direct referralsto appropriate HCRG and partner services for joinedup care.
- Respond to clinical querieswithin agreed timeframes.
- Communicate sensitive informationto families with empathy and escalate concerns when needed.
- Maintain clear, timely communication with service users and professionals.
- Build strong relationships with partner organisations and explain pathways and triage decisions.
- Adhere to safeguarding and organisational policies.
- Ensure children/young peoples voices and parent/carer concerns inform triage decisions.
- Work collaboratively with schools, early years settings, GPs, social care, and mental health/voluntary services.
- Identify and escalate system or process gaps and clinical risks to the Clinical Triage Lead.
- Support EHCNA SOP implementation and related queries.
- Promote effective communication between triage and clinical teams.
- Attend meetings or events as requested by the Clinical Triage Lead.
Qualifications & Experience
- Registered healthcare professional with current NMC/HCPC registration.
- Minimum three years post-registration experience.
