Main duties of the job
As a Band 5 Community Nurse, you will be an essential part of our24hour district nursing service, delivering high-quality, holistic care to patients in their own homes. Working under the direction of the District Nursing Team Manager/Leader, you will:
- Carry delegated responsibility for theassessment, planning, implementation and evaluationof evidencebased nursing care.
- Provide skilled clinical care that supports patients to maintain independence and quality of life at home.
- Supervise and support team membersin the absence of the District Nursing Team Leader when required.
- Work collaboratively with multidisciplinary colleagues to ensure safe, coordinated and personcentred care.
We are looking for someone who is:
- An experienced Registered Nurse with a passion for community healthcare.
- Confident in delivering holistic, evidencebased nursing care.
- Able to work autonomously while remaining a strong team player.
- Skilled in communication, assessment and clinical decisionmaking.
- Flexible, compassionate and committed to delivering high standards of care.
