Main duties of the job
About the Role
As a Band 5 Triage Nurse, you will play a central role in managing the flow of referrals into the District Nursing service. Acting as theDistrict Nurse of the Day, you will:
- Triage incoming referrals, determining clinical priorities, levels of need and risk.
- Coordinate onward referralsto the District Nursing team, ensuring patients receive the right care at the right time.
- Monitor and track all referrals, reducing attrition and supporting seamless continuity of care.
- Work closely with our administrative team to maintain accurate and timely referral information.
- Provide safe, effective decision-making in anoffice-based, nonpatient facing environment.
Were looking for someone who is:
- AnexperiencedRegistered Nurse, qualified to Band 5 level.
- Confident and experienced in clinical decision-making.
- Highly organised, able to prioritise and manage multiple demands.
- A strong communicator who enjoys working collaboratively across teams.
- Motivated to support high-quality community healthcare through robust triage and coordination.
