Main duties of the job
What Youll Do
As a vital member of our Integrated Neighbourhood Team, youll:
- Deliver high-quality, person-centred care to patients in their own homes and residential settings
- Work closely with GP practices and local community services to prevent unnecessary hospital admissions
- Support patients in managing their conditions independently within their own environment
- Collaborate with Nurses, Therapists, Clinical Leads, the Home First Team, and Care Coordinators
