Location: Salisbury District Hospital

Hours: Full time 37.5 Hours per week

An exciting opportunity has arisen for a Senior In-Reach Clinician to join our dedicated team at Salisbury District Hospital.

In this key role, you'll represent HCRG Care Group within the acute hospital setting, supporting effective discharge and providing an admission avoidance service at the hospitals front door (A&E and admission wards).You'll work collaboratively with patients, carers, and internal and external partners to ensure safe, timely, and appropriate discharge pathways.

You'll play a vital part in maintaining patient flow, delivering excellent care, and ensuring a positive experience for every patient.

Main duties of the job

  • Provide a calm, supportive presence on front-door wards to help avoid admissions and support timely discharge.

  • Work collaboratively with social care partners to offer seamless, integrated support.

  • Guide and mentor Band 6 in-reach staff within the acute setting.

  • Help reduce length of stay through early, well-coordinated discharge planning.

  • Join ward whiteboard meetings to maintain progress and assist decision-making.

  • Contribute to or lead key reviews related to discharge decisions.

  • Share updates on discharge policies, national guidance, and best practice.

  • Apply sound understanding of the Mental Capacity Act during planning.

  • Remain vigilant to safeguarding concerns and escalate when needed.

  • Maintain accurate, up-to-date patient records for effective information sharing.

  • Support teams with correct documentation and timely onward referrals.

  • Recognise difficult conversations, offer support, and escalate appropriately.

  • Stay updated on national directives affecting discharge and help teams apply them.

  • Maintain awareness of Health & Safety and Infection Prevention in discharge planning.

  • Work closely with the Patient Flow Hub to resolve barriers to discharge.

  • Participate in system-wide meetings to support onward care planning.

  • Support colleagues who are learning, including those shadowing or newly inducted.

  • Encourage carer involvement and initiate Carers Assessments when needed.

  • Prioritise actions that create safe, effective discharge plans, escalating concerns when required.

Ideal Candidate

Essential

Registered Clinician, and experience of working within health or social care

Ongoing evidence of continuing professional development

Experience of managing staff

Demonstrate understanding of front door services

Experience of working across organisational boundaries within health and social care

Knowledge and understanding of current legislation and best practice in relation to key areas including Information Governance, Care Act, Mental Capacity Act.

Up to date understanding of National Hospital Discharge policies and guidance

Ability to manage, and adapt to change within multiple teams

Ability to contribute to, monitor and implement changes and improvements to services

Knowledge of safeguarding policies and procedures

Ability to interpret clinical information to support risk assessments when planning discharges

Excellent communication skills, both verbal and written, and the ability to influence others in clinical decision making towards discharge Able to change priority, often at short notice

Creative approach to system pressures

Sound conversational skills allowing for difficult conversations with both patients, families, and staff

Ability to remain calm and consider wider picture in particularly stressful situations.

IT proficiency

Ability to prioritise workload, often in response to a changing picture

Ability to work autonomously as well as part of multiple teams

This post will require onsite presence at an acute hospital

Willing to work in other areas of HCRG as and when required to do so

Attendance at and participation in wider system meetings
