Through pioneering use of remote technology we provide acute care from home.
As a Triage Clinician you will be joining our Triage team of Nurses, Occupational Therapists and Care Coordinators.We use cutting-edge telephone technology to effectively triage and manage referrals, providing urgent, immediate, and decisive responses tailored to patient needs.You'll also collaborate with our Virtual Ward and Urgent Care response team, leading the charge in innovative patient care.

This is a fixed-term (6 month contract) part-time (30 hours per week) opportunity.
Main duties of the job
As a Triage clinician you will be a vital part of keeping hospital admissions down but ensuring patients are put-first but all from the comfort of an office setting!
You will:
- Gather patient information and make efficient referrals using telephone, electronic systems, or emails.
- Lead the assessment of a patients condition
- Identify the most appropriate care pathway.
- Use clinical reasoning and knowledge of evidenced-based practice to inform intervention
- Understand and apply knowledge of clinical systems to enable appropriate documentation of clinical records.
- Act as a clinical resource for Care Coordinators and Care Journey Coordinators,
- Maintain accurate systematic documentation of Patient/Health Professional interactions/advice provided.
