Job summary
We are seeking a highly motivatedBand 6 SCPHN Health Visitorto join our team and make a difference in the lives of families in our community.
As a Health Visitor withHCRG Care Group,you will have the opportunity to work with families with young children and provide them with essential health and developmental support. Your role will involve conducting assessments, delivering public health programs, and working closely with other health and social care professionals to ensure positive outcomes for families.
The aim of the service is to reduce health inequalities, safeguard children and improve outcomes.
This role is based with the High Post South Team.
Main duties of the job
Your days will be rewarding and varied and include:
- Public health lead for the health visiting skill mixed team within the defined population/locality area. Delivering the universal reach of the Healthy Child Programme, assessing health needs and identifying and implementing appropriate interventions for a personalised or targeted response utilising the skills of and delegating to the skill mix team in an outcome-focused context.
- To take responsibility for the outcomes of the children within the defined population and collaborate with key partners to address identified public health needs
- Develop and foster effective professional relationships and networks with families and other key partners.
- Provide families and children 0-5 with a service that promotes their physical, mental, emotional health and wellbeing. This will include partnership working with other agencies.
- To promote health and wellbeing through the universal reach of the healthy child programme and signposting. Referring/introducing to partner agencies as required.
- To contribute to multi-agency early support assessment of children and families and take on lead professional role where appropriate.
- To co-manage a caseload within a team of other health visitors acting autonomously and taking responsibility for the outcomes of those allocated for assessment, intervention and management

As a Bank Band 6 Health Visitor, you'll be working as part of the HCRG Care Group Team in our Wiltshire Children's service. As well as the attractive hourly rates of pay, at HCRG Care Group you will benefit from:
- £23.06 an hour.
- Self-booking and access to all bank shifts!Staff are given access to all Bank shifts within their Business Unit and can self-book providing they meet the shift requirements.
- Request a Duty Staff can make shifts or Day Off requests prior to the creation of a roster up to six weeks in advance. This helps promote a better work life balance and allows for personal commitments to be met.
