Job summary

Wilton Community Team are seeking a Band 6 Specialist Community Nurse. Join our dynamic and supportive Wilton Community Team and help shape the future of community healthcare.

As a Specialist Community Nurse, you will:

Provide nursing care, treatment, support, and advice to individuals in their own homes or outpatient settings.

Promote patient independence and wellbeing, working within national and local frameworks, professional guidelines, and organisational policies.

Deliver safe, person-centred care to a diverse caseload of patients with both short-term and long-term conditions.

Take clinical leadership responsibility for peers, junior trained staff, and support workers within your discipline.

Offer expert advice and guidance to other health and social care professionals

Main duties of the job

This post is responsible for:

To manage a caseload of patients independently and be accountable for assessing, interpreting, planning and implementing nursing treatment and care to patients within professional guidelines.

Organise and manage own time and that of junior staff/students, allocating work appropriately in order to achieve the patients goals.

To manage risks in diverse and sometimes challenging environments.

To use evidence-based practice, to ensure clinical care is effective.

To maintain Clinical Governance Standards through audit, research and evidence gathering.

Assist in the monitoring and protecting of groups and individuals whose health and well-being may be at risk e.g.: vulnerable adults.
