Are you passionate about delivering high-quality healthcare in the community? Were excited to offer a fantastic opportunity for the role of a Band 5 Community Nurse to join our team within Adult Community Services in Surrey.
This role covers theSurrey Heath locality, and you will be expected to travel across the area as required, therefor having a driving licence and access to a vehicle is essential
The community nursing team works 8am to 8pm, 365 days a year. Our shifts include a mixture of 8am to 4pm, 9am to 5pm and 12pm to 8pm. You would only do one late shift a week and join the rota to work one weekend in four.
This position would be suitable for a student nurse about to qualify who would like to work in the community. We offer a robust and supportive preceptorship programme for your first year, where you can develop into a confident and skilled community nurse.
Main duties of the job
Within this role, you will:
- Undertaking first line assessment, care planning and evaluation of patients care needs and takes action to address underlying causes eg nutrition assessment, biomechanical asessments.
- Manage referral to the community nursing service , assessing the priority of need appropriately and adhering to criteria to treat.
- Works with the muliti-disciplinary team to support the managing of people with long term conditions, palliative care and terminal illnesses.
- Plans, delivers and evaluates health promotion strategies and advice to empower individuals
- Utilises evidence based practice and research to inform patient care.
- To be responsible for the caseload in the absence of the District Nurse for short periods with the support of a named District Nurse.
- To participate and lead clinical audits and clincal governance agenda, implement change in clinical practice and support monitoring and evaluation.
- To implement, monitor and maintain agreed standards of care, reporting to the District Nurse in all instances where such standards cannot be met.
