Job summary
As a Community Nurse Band 5 you will join our innovative and forward-thinking team of Community Nurses and Healthcare Assistants, where your growth and development truly matter. As community services evolve, we remain committed to a responsive, patient-centred approach. You'll collaborate closely with a range of services and care for a diverse group of patients, helping deliver the right care, in the right place, at the right time.
We prioritise flexible, agile working while maintaining high-quality care, offering a 7-day service from 08:00 to 22:00. This role allows you to rotate across our community teams, including urgent response, community nursing, and Hospital at Home giving you the chance to broaden your experience and clinical skills.

Main duties of the job
As a Community Nurse Band 5 you will be responsible for:
- Delivering evidence-based nursing care, including assessment, care planning, and record maintenance.
- Supporting the leadership and organization of the community nursing team.
- Providing person-centered care while ensuring service user autonomy and choice.
- Maintaining compliance with clinical, legal, and ethical standards

Essential
Registered Nurse (Adult) or Paramedic with current NMC or HCPC registration
Ongoing professional development relevant to a Band 5 role
Confident in using structured approaches to assess, plan, and evaluate care
Versatile clinical experience suitable for a community setting
Strong organisational, communication, and interpersonal skills
Committed to delivering needs-led, patient-centred care
Desirable
Confident in assessing, planning, and evaluating needs-led care
Broad clinical experience suited to community healthcare
