Clinical Team Lead for Community Rehabilitation

Job summary

We are looking for a Clinical Team Lead to join our Community Rehabilitation team based in the Orbital Swindon.

As a band 7 Clinical Team Lead for Community Rehabilitation, you will work closely with the Head of Intermediate Care Therapies and Therapy Service and Team Leads to ensure the delivery of Community Rehabilitation services. You will provide effective rehabilitation within community services to promote a caring, compassionate and responsive therapy service to the population of Swindon, through management and leadership of the Community Rehabilitation Team which includes Acute Care at Home, Reablement, Bed-based, and Home-based services. You will develop effective working relationships with staff to promote a supportive environment in which they can develop and learn.

Main duties of the job

In this inspiring leadership role, you'll work closely with local Commissioners, GPs, social care teams, and voluntary sector partners to support a smooth and effective patient journey across all levels of care. Central to your role will be providing clinical leadership to the Community Rehabilitation Team ensuring high-quality care, timely patient flow, and strong alignment with NICE guidelines.

As a compassionate and forward-thinking leader, you'll help shape service development, drive innovation, and maintain high standards through collaboration with patients, staff, and wider stakeholders. Using data and insights, you'll contribute to strategic planning and continually strive to enhance patient outcomes.

You'll lead with integrity supporting governance, fostering a culture of learning, and guiding staff through clinical decisions and safeguarding practices. While not a budget holder, you'll ensure resources are well-managed and workforce targets are met.

Your commitment to team development will be key, from appraisals and training to performance management and change support. With a focus on resilience, consistency, and compassion, you'll help deliver a trusted service that meets the needs of a diverse community operating across Swindon and surrounding areas, seven days a week.

The Ideal Candidate

Essential

A confident, credible, and resilient AHP leader with active registration and postgraduate-level education or equivalent experience. Brings a strong foundation in leadership, supported by formal training or ongoing development, and a proven track record in team leadership, HR, resource management, and service improvement. Skilled in clinical governance, investigations, and managing change, with sound knowledge of finance, performance, and workforce planning. Deeply familiar with community services, national agendas, and GWH Trust practices. A collaborative, compassionate professional who communicates effectively, thrives under pressure, and consistently puts patient care first motivating and inspiring teams with integrity and empathy.

Desirable

Brings experience in leading projects with confidence and clarity, supported by a formal leadership qualification. Demonstrates a high level of emotional intelligence, enabling strong, empathetic connections and effective team dynamics. Comfortable networking across diverse settings and skilled in preparing and delivering engaging presentations to various audiences.

Other requirements:

Flexible and supportive in approach, with a willingness to work across different areas of the Trust as needed. A confident driver with the ability to travel between sites, and comfortable using IT systems to support effective and efficient working.
