Job summary

As a Band 6 Physiotherapist you will provide a high standard of physiotherapy assessment, treatment and advice to patients/clients, carers and families. Wherever possible avoid hospital admission and reduce in-patient length of stay to ensure that patients receive the appropriate care in the most appropriate setting. Undertake the lead role in the assessment, diagnosis and formulation of physiotherapy treatment plans across all the different service areas within CRT such as Home based services, Discharge to Assess beds, Woodlands Therapy Centre and to undertake specialist physiotherapy assessments for patients presenting with complex needs eg Movement Disorders, MND, Vestibular pathology, and people who are frail and falling.

Main duties of the job

  • Take full professional and legal responsibility for clinical practice and caseloads.

  • Use advanced clinical judgment to assess, prioritise, and refer rehabilitation cases appropriately.

  • Factor in home environments and social support when planning care and ensuring therapist safety.

  • Lead physiotherapy input within interdisciplinary teams, providing expert advice and guidance.

  • Contribute to audits and governance to support continuous service improvement.

  • Offer specialist support and education to colleagues, students, and support staff.

  • Supervise and mentor junior team members and students.

  • Work with external services to promote patient independence at home.

  • Conduct thorough assessments for patients with complex needs, delivering accurate diagnoses through advanced clinical reasoning.

  • Develop person-centred care plans by integrating varied information and setting collaborative, evidence-based goals.

  • Ensure informed consent and act within legal capacity frameworks.

  • Monitor progress and adapt treatment plans to optimise outcomes.

  • Coordinate with health and social care teams, carers, and voluntary agencies to deliver cohesive care.

  • Participate in case conferences and interdisciplinary meetings.

  • Maintain high care standards through quality monitoring and service development.

  • Keep accurate clinical records and submit timely reports as required.

  • Stay engaged with safeguarding practices for vulnerable adults.

Essential:

Qualified Physiotherapist with a degree in Physiotherapy and full HCPC registration, committed to continuous professional development in areas including neurology, elderly rehabilitation, and falls prevention. Brings broad rotational experience across clinical settings, with strong clinical reasoning, caseload management, and decision-making skills. Confident working independently or as part of a team, and experienced in supervising and supporting junior staff.

Demonstrates excellent communication and report-writing abilities, with a compassionate, patient-centered approach that builds trust. Skilled in data interpretation and reflective practice to support evidence-based care. Fully aware of safeguarding responsibilities, with a sound understanding of local procedures. Flexible, mobile, and adaptable to changing demands within the Trust.

Desirable:

Proficient in IT, with formal qualifications and strong digital literacy. Qualified NVQ/Apprenticeship Assessor with experience in staff development and team leadership, supported by basic management training. Completed advanced clinical training in areas such as Normal Movement and Vestibular Rehabilitation, with expertise in managing complex caseloads and delivering a wide range of interventions.

Experienced in service development projects, with a good understanding of resource management and cost-effective care. Confident in using both standardised and informal assessments to guide clinical decisions. Strong interdisciplinary team player with a background in Community Urgent Care, Discharge to Assess, Reablement, and Falls Service rehabilitation. Aware of team dynamics and skilled in supporting constructive conflict resolution.
