Look no further. We are developing our services and our team of Rehabilitation Assistants. You will be at the forefront of developing quality services, and you will be key to shaping and redesigning how we deliver our innovative services to patients across the community pathways.
Working in an inter-disciplinary team, the post holder will assist the qualified members of SCHS Community Rehab Team with the professional and clinical duties and with the day to day organisation, ensuring a high standard of care to our client group. Working within the interdisciplinary team you will prevent unnecessary admissions for patients, support earlier discharge home from bed based facilities and share with the therapists, the overall therapeutic aim i.e. to enable patients to work towards their potential level of physical, mental and social independence and support carers in their role.
Main duties of the job
In a rehabilitation assistant role, you'll assist therapists and nurses by carrying out delegated tasks, simple assessments, and interventions in patients' homes, community settings, or unit-based venues. You'll observe and report any changes in patient condition promptly, ensuring care remains responsive and effective.
Working closely with clinical teams, you'll contribute to patient assessment and treatment, drawing on shared information and guidance. At times, you'll work independently visiting homes to fit equipment, carry out follow-ups, or support safe practice while managing personal and clinical risks appropriately.
You may also support joint visits, help measure for equipment such as rails, and provide follow-up calls to ensure continuity of care.

Ability to teach practical skills to patients/clients and new staff within the department.
