Job summary
We are seeking a dedicated and compassionate Child and Family Wellbeing Practitioner to join our team and support our Health Visitors in delivering the Healthy Child Programme (0-5), ensuring positive outcomes for families. This rewarding role will see you working closely with families, providing feedback on outcomes, and supporting the facilitation of local healthy family drop-ins. Additionally, you will plan, deliver, and evaluate virtual health promotion sessions aimed at improving family wellbeing.
This is a full-time position (37.5 hours per week), though we are open to considering applicants who prefer to work 30 hours per week. The role is based at our Derby Court location in Trowbridge. A valid UK driving license and access to a car for work-related travel are essential.
Main duties of the job
As a Child and Family Wellbeing Practitioner, you will:
- Work directly with families, under the guidance of Health Visitors, to deliver support and feedback on outcomes.
- Assist in facilitating local healthy family drop-ins to promote family health and wellbeing.
- Plan, deliver, and evaluate virtual health promotion sessions, leading discussions with groups of parents and carers.
- Conduct Ages and Stages 27-month reviews, incorporating the Early Sign Language Identification Measure tool.
- Provide evidence-based Support Bundles to families, under the direction of the Health Visitor.
- Ensure effective communication and appropriate information sharing with partner agencies.
- Collaborate with Health Visitors to assess child development and growth.
