Job summary
We have an exciting opportunity to join our Keynsham service as a Band 6 SeniorCommunity Nurse based at our Keynsham Health Centre location.
You will join a caring and supportive team of Community Nurses and Healthcare Assistants where individual development and progression is key and teamwork is at the centre. You will also work alongside the Reablement team, Community Matrons and specialist nurses in a multi-disciplinary atmosphere. The service is located right at the end ofthe high-street with a range of shops and Sainsburys and Tesco situated nearby. The service also has its own dedicated car park.
The service hours typically being8.00am - 5pm Monday to Fridaywith one in every 4 weekends. Your focus will be to offer advice and support to housebound service users in their own homes to promote their well-being and independence. You will also be offered a flexible role where you can work your own diary to provide continuity of care. There may also be the opportunity to support in a weekly leg ulcer clinic.

Main duties of the job
- responsible for contributing to the holistic assessment, planning delivery and evaluation of care to patients within their own homes.

- deliver our high service standards, sharing bestpractice, and actively improving the way we work.
- The purpose of the role is to manage the district nursing team. The post holder has overall responsibility for all aspects of service user management in the team, planning day to day care and problem solving for the team where necessary, influencing changes to clinical practice and being a role model. The role involves communicating with the multidisciplinary team and attending MDT meetings
