Main duties of the job
As a Community DSN, you will:
Deliver specialist care and tailored management plans to adults with diabetes in the community, including those with complex physical and psychosocial needs.
Empower patients to manage their condition through structured education and self-management strategies.
Act as a resource and educator for healthcare professionals, promoting best practices in diabetes management.
Collaborate with multidisciplinary teams to provide seamless, integrated care.
Contribute to service development, quality improvement initiatives, and audits to maintain our award-winning standards.
Promote healthy lifestyle changes and prevent diabetes-related complications through community outreach and education.
