Job summary
The Melksham and Bradford on Avon Team are looking for a new team manager. The successful candidate will be required to work with, lead and develop an experienced, passionate group of healthcare professionals. The team comprise of therapists, nurses, support workers, administrators and clinical care coordinators. Your leadership team will include a clinical lead for therapy and clinical lead for nursing. Your operational leadership will be supported by the Clinical Leads governance oversight in clinical care. Our community team support patients with a wide range of conditions both in their own homes and within our community hospital. As the team manager you will work closely with your neighboring team manager. We provide a buddy system for both day to day operational demands but also peer support. Main duties of the job
To provide support to the team via line management in line with the supervision policy.
To provide effective line management support to the Clinical and AHP Band 7 staff plus allocated Band 6 staff within the community team.
To continue to hold a clinical caseload balanced with Team Leader responsibilities, using this clinical responsibility to model best practice and willingness to demonstrate adaption of new processes and ideas
To work in partnership with local commissioners, GPs, Social Care and Flow hub colleagues to ensure the teams deliver and support the effective movement of patients between primary community intermediate and secondary care.
To work closely with other teams in order to ensure effective care pathways and information sharing
To work with colleagues in other teams to increase capacity by up-skilling
To coordinate staff recruitment, appointments and terminations within the team.
To ensure care pathways are followed; flagging any variance required to support further development
To keep clinical records in line with records keeping policy and support the team to do so.
To ensure all safeguarding policies and SOPs are adhered to including ensuring access to supervision is kept as stipulated.
To keep quality and safety training up to date and monitor this in the team.
