A Band 5 School Staff Nurseopportunity has arisen working for HCRG Care Group, Wiltshire, as part of the Wiltshire wide School Nursing Team.
You will be a Registered Nurse with an interest in providing services to Children, Young People and their families within the community setting. Youll join an experiencedteam who are focused on clinical excellence.
We are very excited to be deliveringour SchoolNursing Public Health Service to providethe Healthy Child Programmeusinga holistic approach in order to ensurea comprehensive Public Health Service to the school aged population and their families.
The serviceembraceselectronic record keeping, mobile working and aSingle Point of Access (SPA) for all services delivered tothe 0-19 population, providing one record for one child.
Working within the Derby Court Team, you will beparta team of other Staff Nurses and School Health Support Workers, led by Specialist Community Public Health School Nurses, working closely with other SCPHN's, Schools, and all other community services supporting the 0-19 population, such as Health Visitors, Community Nursery Nurses,Speech and Language Therapists, and Community Paediatricians.
Main duties of the job
As a School Staff Nurse you will:
- Support allocated caseload of vulnerable children and adolescents through proactive discussions with families, schools, other agencies, attendance at Child Protection Conferences and one to one discussions
- Workacross organizational boundaries to contribute towards the identification of population health needs
- Collaborate with key partners to address public health needs as an advocate for children/young people, families/carers and the school health service
- Be responsible for the assessment of childrens health needs to identify any deviation from normal development
- Promote healthy lifestyles through opportunistic discussions and health education
- Support children and adolescents with chronic and complex health needs by following current policy, processes, and pathways.
