An exciting opportunity has arisen for a Children's Learning Disability Nurse to join the WiltshireLearning Disability Nursing Service.The successful candidate will have a experience in Learning Disability Nursing and be a Registered Learning DisabilityNurse
Working closely with ourLearning Disability Nurses and Support Workers across Wiltshire, youllmanage an allocated caseload of children, young people and familiesindependently, co-ordinate and lead the assessment, planning, implementation and evaluation of care. You will need to engage the involvement of families in care and service planning and ensure practice is excellent evidenced based, effective and up to date with collaborative working.
Your case load will be in Wiltshire. We have hubs based in Chippenham, Salisbury and Trowbridge. You will be located at the hub closest to your home.
Youll care forpatients and their families in the way thatyou believe is best, delivering our high service standards, sharing bestpractice, and actively improving the way we work. You will need to raise all care matters relating to the child and family and follow all safeguarding procedures.
