Job summary
HCRG Care Group are proud to have an exciting position to work as part of the dynamic childrens continuing care team (CCCT) to help deliver the packages of care that have been commissioned to our team. The service runs 365 days per year.
You will work within and across professional and organisational boundaries, liaising with a range of professionals, statutory and voluntary agencies to deliver individualised care planning and care packages
The post holder will require a solid understanding of the concept of clinical governance and be skilled in the identification, assessment and planning of care to children who meet childrens continuing care criteria.
Exhibiting excellence in the standards for childrens nursing the successful post holder will be an effective teacher and role model who demonstrates our HCRG care group values and behaviours.
Main duties of the job
- You will worka mixture of clinical shifts to ensure the needs of the care package are met. Including working nights, weekends, evenings, and day hours.
- The post holder will be a point of contact for the young people and families on the CCCT and act as a liaison between professionals from all agencies when appropriate.
- To participate in the development of service specific protocols and guidelines which are based on sound research findings and demonstrate best practice
- Provide on call support for our staff who are working unsocial hours, generally providing on call 5pm 9am for one week in every 6. This will be paid at the AfC on call rate.
- Work in partnership with both the child/young person and their family/carers in assessing, planning, implementing and monitoring/updating the plan of care.
