Job summary
We are looking for an experienced Band 6 Nurse (SCPHN desirable) to join our Children in Care health team for22.5 hours a weekbased atDerby Court, Trowbridge,though this is a community role.
The Children in Care health team, work with our partnership agencies striving to make positive changes and improve health outcomes for children in care.
You will join a supportive and welcoming team of specialist health nurses and admin support and be managed by the Named Nurse for Looked After Children. The Team is committed to professional development and successful applicants will have opportunities to access further training.
Working hours are typicallyMonday-Friday 9am-5pm, though we are committed to flexibility with hours and offer hybrid working to meet the needs of the staff, clients, and the service.

Main duties of the job
As a Children in Care Nurse Specialist you will work with wider team to provide a service that promotes the physical, mental, and emotional health of children and young people and support for families and carers.
You will:
- Assess, plan, and evaluate the health needs of children on the caseload, challenging and escalating concerns where needed.
- Undertake assessments, considering emotional, mental, physical, and sexual health, ethnic and cultural needs.
- Provide direct clinical services to Looked After Children, to ensure that where possible every child has an individual and holistic health care plan.
- Provide direct services, enhancing local co-ordination and communication between young people, carers, health, education, social services, and voluntary agencies.
- Provide quality care and health assessments making appropriate care planning recommendations, surveillance, advice, support and health promotion
- Deliver training to foster carers and other health and social care professionals.

- Be a Registered Nurse withNMCregistration
- Hold a SCPHN Qualification (desirable)
- A fullUK Drivers Licenceand access to a car.

- It would be an advantage if you held a qualification/ knowledge in Contraception and Sexual Health due to the services strong focus on young people's sexual health
