Job summary
We have an exciting opportunity for an enthusiastic and motivatedBand 6Dieticianto joinour new Early Supported Discharge service, working withour dynamic team at Gravesham Community Hospital. As a member of the Early Supported Discharge Team for Stroke patients and other Neurological conditions, you will play a crucial role in delivering high-quality care within the community setting. This is an excellent opportunity for a passionate Dieticianto make a real impact in the lives of patients and contribute to the ongoing development of our service.
This is afull timeposition. The Early Supported Discharge service operates8am - 4pm Monday - Saturday.
A full driving licence and access to a vehicle is essential in order to carry out community visits to patients in their homes and visit other bases.
Main duties of the job
- To deliver a service for people with disorders of diet, communication and swallowing following a stroke within the Early Supported Discharge framework.
- To work as part of an integrated multi-disciplinary team of staff in a community setting including community rehab wards
- To undertake advanced diagnosis, assessment and treatment of patients with a range of dietary needs
- To proactively develop the Dietetic service for patients within the stroke pathway
- To contribute to the development of junior therapists and rehab assistants within the wider community teams
- To liaise and offer highly specialist advice as appropriate to a wide range of professionals
- To be responsible for the daily organisation of sessions and caseload
- To contribute to the triage of referrals
- To provide advice, support and education for patients and their families/carers
