Job summary

HCRG Care Group are looking for a band 4 care co-ordinator to join our developing and innovative reablement service in Bath and North East Somerset.

The Reablement service provides vital short-term therapy input to people by helping them to regain or attain as much independence as they can in their own homes.

The successful post holder will be based within the North-East Somerset Team and will be based at St Martin's Hospital, Bath.

The role of care co-ordinator is crucial in supporting the co-ordination of support visits across the locality area and North East Somerset, and also includes patient-facing responsibilities including care plan reviews and participation in MDT discussions.

The reablement service operates 7 days a week throughout the year, between the hours of 08.00 and 22.00.

As the service operates seven-days a week, all staff are expected to work weekends and bank holidays within a team rota. The post holder will be expected to have contact with service users and consequently the work may involve travelling within the Bath & N E Somerset area.

Main duties of the job

As a care coordinator you will:

  • Respond to referrals promptly and add support plans onto the electronic scheduling system and patient records.
  • Co-ordinate and supervise the work of the RTW colleagues. This may include allocating service users, covering absences (due to sickness and AL), expense claims, monitoring performance including completing staff supervisions and appraisals, and service delivery, communicating with clinicians to respond to emergencies, dealing with complaints, etc.
  • Take part in an internal on-call duty rota for the service (which covers 7 days a week 7am 10pm).
  • Provide comprehensive advice and information to service users on the range of services available to them/ issues affecting them e.g. benefits, leisure, etc.
  • Conduct goal/ support plan reviews.

You will have experience of working within a care setting e.g care home or rehabilitation and have knowledge and experience of the importance of maintaining independence.

You will have an awareness of the legislation affecting health and social care and the role of the Care Quality Commission.

You will have strong communications skills, both written and verbal, and be able to problem solve under pressure.

You will be able to work collaboratively as part of a team, including re-prioritising of workload when required.

You will be able to co-ordinate a team of rehab therapy workers, including support with assigning tasks, motivating and developing skills.
