Job summary
We have an exciting opportunity for an experienced Band 7 to join our Urgent Community Response (UCR) Team as a Clinical Practice Lead.
The team ensures that crisis response care is available to all people within their homes or usual place of residence, including care/residential homes.
The role will be a Full Time, Band 7 with Agenda for Change terms and conditions including NHS pension. The service is open 365 days a year and is operational between the hours of 8am 8pm.
Main duties of the job
Responsibilities include but are not limited to:
- Ensuring colleagues who work in the service, in and out of hours develop sound clinical skills suitable for their level of experience and job role.
- Provide clinical support to Registered Nurses and Support workers in the UCR/Virtual Ward (VW) service
- To support the triage & prioritisation of the UCR/VW workstream
- Engage in effective communication with patients, relatives, carers, and other agencies directly involved with provision of care, having established links to a range of communication skills to ensure effective communication with those who may be hard of hearing, of ethnic origin or with learning difficulties.
- Act as a clinical supervisor supporting clinical supervision for UCR nursing colleagues.

We are looking for someone who can act as a positive role model and be visible to your team, demonstrating effective communication, leadership styles and behaviours. You will be able to work well in stressful situations whilst showing understanding and compassion.
You will have previous experience in a Band 7 role, working in a team effectively. The ability to make clinical decisions associated with high clinical risk and potential harm to patients.
A full UK driving licence is required for this role and access to a vehicle.
