Job summary
We are looking for a highly motivated and enthusiastic nurse with a special interest in cardiology to join the Community Cardiac Rehabilitation Team in Swale (Sittingbourne and Sheppey). The successful candidate will join our multidisciplinary team, working in collaboration to motivate and inspire patients and their carers to engage in improving their health and recovery following a cardiac event.
We provide high quality care which is focused on improving patients health and quality of life through health education, advice on cardiovascular risk reduction, physical activity and stress management as recommended in the BACPR standards. The successful candidate would work in both inpatient and outpatient setting across phases 1-3 of the cardiac rehabilitation programme. The successful candidate will also offer support to the Cardiac Specialist Nurses who manage patients with LVSD.
The post holder will require at least 2 years post qualification experience and to have worked within the cardiac speciality. As part of a small dynamic team, you will have regular supervision, training, and support from highly skilled colleagues to address your learning needs and will work closely with members of the MDT. Main duties of the job
As a Cardiac Nurse you will
- Be responsible for managing a defined caseload of patients, using theoretical knowledge and high levels of practical experience to formulate specialised programmes of care.
- To manage a clinical caseload of cardiac patients. Taking responsibility for direct patient care, assessing, planning, providing, and evaluating nursing care needs including monitoring, diagnostics, and investigations.
- Managing a caseload of cardiac rehabilitation patients within a community setting
- Responsible for teaching patients and their carers to manage their cardiac conditions to the best of their abilities. The aim being to prevent short term and long-term complications, this may be one to one or in groups.
- Communicates effectively with patients and carers to ensure a clear understanding of cardiac conditions and management. This will involve providing and receiving highly complex, highly sensitive information and will require developed persuasive, motivational, empathetic and reassurance skills. There will be barriers to understanding.
- Accept referrals from health professionals involved in caring for people with cardiac conditions. Responsible for formulating detailed assessments of the patients physical, social and psychological needs.
- Be responsible for supporting optimising drug therapy and supporting and guiding patients to make lifestyle changes.
