Job summary

Were looking for Nurses tojoin our North Kent Rapid Response Servicewith a base at Gravesham Community Hospital, Gravesend to help us continue to change things for the better. The Rapid response team works across 7 days a week from 8am - 8pm.

We are looking for a registered nurse who has had experience in either community, or a ward based environment. If you are currently a student looking to take your first step into healthcare we can look at how we can support you with our preceptorship programme as well.

Main duties of the job

Within this role, you will:

  • Manage referrals and caseload, for a 7 day per week Rapid Response pathway
  • Assist the Team Leader in running a team, deputizing for the Team Leader in their absence.
  • Actively work as a member of the Rapid Response Pathway to provide high quality nursing care to clients within a community setting, preventing unnecessary hospital admissions and supporting hospital discharges.
  • Be responsible for contributing to the holistic assessment, planning delivery and evaluation of care to patients within their own homes.
  • Implement, monitor and maintain high standards of care delivery at all times.
  • Care for patientsand their families in the way thatyou believe is best.
  • Deliver our high service standards, sharing bestpractice, and actively improving the way we work.

You will be a NMC/1stlevel registered Nurse, you will have the ability to work both independently and also as part of a wider multidisciplinary team. It is expected that youshould have strongcommunication and organisation skills and a flexible approach which will ensureyou deliver positive experiences to patients at every stage of theirhealthcare journey. You will have a full UK driving license with access to a car for work purposes.

You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
