Job summary

Are you an enthusiastic and passionate Nurse looking to join an organisation that is dedicated to clinical excellence whilst ensuring our staff members feel valued each and every day?

We are looking for Nurses, Occupational Therapists and Physiotherapists to join and expand our recently transformed Rapid response team. This is an exciting opportunity to be involved in creating an efficient service focusing on admission avoidance and supporting early discharge in line with the NHS 10 year plan.

The service consists of a team of approx. 33 staff in total whom work in close collaboration with the intermediate care teams and community nursing teams. The service operates 7 days a week, including bank holidays, 8am until 8pm with various shift patterns available. We have a robust induction programme that includes relevant training and protected consolidation time, which successful candidates would undertake in their first 6 weeks in role. Following induction our supervision and appraisal process will support applicants with further accredited development.

Main duties of the job

As a Nurse within HCRG Care Group Swale rapid response team youll join a team who have recently been through transformation to create a fully multidisciplinary 2hr response service. Through the pandemic we have continued to provide a high level of service and have utilised innovative tools to enable mobile working and virtual assessments where appropriate.

Your days will be fast paced and varied and include offering support to patients and families whom are in crisis or acutely unwell, in order to prevent a hospital admission. Also supporting early hospital discharges by offering clinical assessment and treatment within their own home as opposed to a lengthy acute hospital stay. Advanced clinical assessments skills will be greatly utilised within this rapid response role.

You will have a degree qualification and registration with relevant body (NMC) for this role. You will have a full UK driving license with access to a car for work purposes.
