A fantastic opportunity has arisen to join one of the largest expert nursing teams in the UK to deliver first class specialist stoma care in the community. Whether you are a Stoma Care Specialist or a Nurse / District Nurse with a desire to learn a specialised skill we are looking for someone who will be passionate in supporting a recognised care pathway of stoma patients from the acute into the community.
Location:Ealing, Brent and Harrow
Contract:Permanent, working Monday to Friday (with occasional weekend on-call, no more than three occasions in one year)
Main duties of the job
Responsibilities:
The Nurse Specialist will develop and deliver a community-based stoma care service in patients own homes as part of a secured partnership completing the following:
- Support to patients discharged with a stoma from the assigned hospitals and surrounding area in support of the Acute based teams
- Provide safe, effective and specialist nursing care, information and advice to patients and their relatives
- Teach and support other nursing and non-nursing staff and carers as required
- Provide specialist clinical advice on the safe provision of a stoma care nursing service
- Foster good communication links and co-operation between GPs, Community Nurses, Ward Managers and other disciplines / departments
- Ensure timely and accurate entry of data into appropriate patient record keeping systems that meet the requirements of data protection
- Support the introduction of new products and provide opportunities for end users to review
- Participate in discussion and the operation of research programmes affecting patients / the service
- Work with Company Nurse Manager to set annual clinical objectives and track regularly to ensure timely completion
Person Specification:
- Qualified Nurse RGN (essential)
- Community / district nurse experience (desirable)
- Recognized Stoma Care Qualification (desirable)
- Car driver essential (home visits will be required)
- Proven experience with an interest within the stoma care / colorectal area
- Evidence of recent post registration training and development
