Job summary

Predominantly based in the Bristol or Gloucester(Tetbury) branch, you will be the named nurse for a specified number of Adults.You may also be responsible for supporting additional branches with their complex care packages on occasion.

We are looking for an enthusiastic and forward-thinking community Adult Nurse, ideally with community experience and a teaching qualification or experience, although we will consider those with proven expertise and sound clinical skills in general nursing who are keen to gain community experience. You will have the confidence and competence to oversee a varied case-load, and contribute to the on-going development of nurse-led care provision and support to non-registrant professional carers delivering care.

Main duties of the job

Reporting to the Branch Manager(Operational) and the Mobilisation Complex Nurse Specialist(Clinical Supervision) your role will include:

  • Responsibility for contributing to the holistic assessment, planning, delivery and evaluation of complex clinical care packages
  • Implementing our high standards of care
  • Participating in quality improvements through supervision and audit
  • Re-assessing and re-evaluating programmes of care when needed.
  • Supporting non registrant carers to provide hands-on care; this involves teaching and undertaking competency assessments/ sign-offs.
  • Being an active member of the nursing team developing teaching tools, policy and sharing best practice whenever possible.
  • Contributing to company-wide initiatives and improvement projects relating to clinical care provision.
  • Identifying and developing new business streams

We are looking for a dynamic and independent clinical professional with solid Adult Nursing experience either as a specialism or dual trained and a flexible, enthusiastic, and can-do attitude to join our West Region Team. The role is part-time working. (28 hours to per week, over 4 working days). The successful candidate can either be located in our Bristol or Gloucester Branch (Tetbury) and will need to support clients across the Bath, Bristol and Gloucester area.

ROLE PURPOSE

The role of a Regional Nurse is to provide clinical expertise and professional leadership to specific branches within a designated region, to enable the planning and delivery of safe and high-quality complex care provision to adults in their own homes and community settings.

  • To be accountable for and lead on the clinical components of care delivery
  • To risk assess individual client referrals and to continually provide clinical input, expertise and advice relating to care-planning and delivery across the pathway and evaluate outcomes
  • Attend meetings and/or liaise with MDT, commissioners, case managers and clients etc to build and manage clinical caseload
