Job summary

We are looking for a Practice Nurse (RGN) with a passion for delivering outstanding care.

Youll be working 32 hours a week (4 x 8 hour shifts), preferably Tuesday to Friday at Woking Lodge.

Cygnet Lodge Woking is a 31 bed service providing acute and high dependency rehabilitation services for men with complex mental health needs.

We have 3 different sections...

- Marlowe Ward & Milligan House High Dependency Inpatient Rehabilitation service for men

- George Willard Ward Acute Service for men

Main duties of the job

Your day to day

  • Monitoring the physical health of patients within Cygnet Lodge Woking
  • Work collaboratively with the clinical team to meet the needs of patients and supporting the delivery of policies and procedures
  • Being the lead for physical health care at Cygnet Lodge Woking
  • Monitoring of physical health conditions, health promotion and screening activities
  • Co-ordinating the GP appointments on a weekly basis
  • Responsible for referring patients if other health professional involvement as indicated
  • Ensuring documentation is completed i.e. GP notes, physical health monitoring forms and Physical Health Care Plans
  • Liaising with the teams, responding to the educational needs of patients and becoming involved in the training of staff as required Identify and manage as appropriate, care and treatment plans for patients at risk of developing physical health conditions
  • Co-ordinate the Blood Clinic on a weekly basis inc. Clozaril and Lithium monitoring
  • Closely monitor patient health issues such as obesity, hypertension, diabetes, epilepsy, asthma, COPD, minor injuries and illnesses

An experienced RGN with a current Pin Number, youll have

Essential:

  • The ability to maintain documentation, undertake audits & demonstrate integrity in your role
  • Take own responsibility for completing mandatory training and CPDs as required for the role.
  • Ability to identify and manage as appropriate, care and treatment plans for patients at risk of developing physical health conditions
  • Compassion, resilience & a dedication to empower & support service user independence

Desirable:

  • Post registration working Experience (minimum 1 year)
  • An interest in leadership, research & assessment
  • Dual Qualified(RMN and RGN)
