The post holder will lead on investigations and work closely with service users and families to focus the organisation on learning lessons from incidents. This role requires attention to detail, consideration of impact and ability to focus on pathways and experiences of people who use our services.

A major focus is on potential medical malpractice and general liability claims at Cygnet Bury. Assisting in identifying measures to improve patient safety and quality of care and decrease/limit risk through lessons learnt from incidents, investigations and findings.

Main duties of the job

Your Day to Day:

  • To be responsible for applying the risk management process of identifying risks, setting priorities, and treating the risks through elimination, prevention, reduction and transferral.
  • To be responsible for identifying incidents with potential for liability and financial loss, using loss prevention monitors, including occurrence (incident reports), focused occurrences and patient complaints.
  • Conducting investigations of incidents/ adverse events with quality of care, systems, regulatory and/or potential liability questions and concerns.
  • Review daily occurrences through the online incident management system (Datix), aimed at identifying significant events, as well as patterns and trends, in order to identify and facilitate appropriate risk management and quality improvement interventions.

  • Proven ability to function independently and communicate effectively at all levels of an organisation required.
  • Must have strong organisational and analytical skills.
  • Learning from serious incidents and investigations is essential for our organisation, team and personal development.

Successful candidates will be required to undergo an enhanced DBS check.
