Primary Integrated Community Services are supporting Bassetlaw Hospice with the recruitment for the following vacancy.
The
post holder will have a service wide remit to support services to deliver excellent
palliative and end of life care throughout In-Patient Unit (IPU) Community
Services (CNS Team) and the Day services/ Wellbeing services. This post holder
will also be expected to deliver teaching and supervision to the Advanced
Clinical Practitioners (ACPs) and Clinical Nurse Specialist (CNS)
The
Service operates throughout Bassetlaw, providing palliative and end of life
care and services to all patients registered with a Bassetlaw GP, with
consideration to out of area patients on an individual basis.
This
post holder will report directly into the Director of Clinical Services, and
access supervision via the GMC. The post holder will create networks with other
hospice and palliative care colleagues to strengthen the alliances and
knowledge base for the benefit of the patients and those
Were hiring this role as an employed member of staff, an a fixed term contract for two years. You'll be part of the team, on payroll, with all the support and benefits that come with it. Not on a self-employed basis.
This role is offered on a 4-day per week basis however this can be negotiable.
Main duties of the job
KEY TASKS
- Maintenance of high quality, evidence-based,
Specialist Palliative Care to patients and their families with care, compassion
and dignity

Responsibility for the day-to-day
clinical management of patients including hospice in-patients and community outpatients

To assist in the set up and particulars
of a forward thinking out of hours medical model for the hospice.

Provide consultant level medical
leadership within Specialist Palliative Care service

Participate in departmental Clinical
governance and quality improvement

Work to achieve personal and Hospice
objectives

Ensure active participation in
continuing professional development and engage in appraisal and revalidation

About us
Bassetlaw
Hospice is a newly commissioned provider of palliative and end of life
services. Whilst Bassetlaw Hospice has been open for over 30 years, the service
has been delivered through Nottinghamshire Healthcare Trust for the past few
years, but it is now an independent charity run service with funding via the
ICB
Bassetlaw Hospice, Retford, North Nottinghamshire has 6 inpatient beds supported clinically by established Advance Clinical Practitioner roles. It also provides clinical nurse specialist support to the community as well as day therapy and outpatient clinics. There are excellent links with local secondary care, primary care and ICB colleagues as well as aspects of palliative care delivered via the Nottinghamshire Healthcare Trust community Services.

Duties of the
Post
As Consultant in Palliative
Medicine you will provide the following:

Deliver consultant level day-to-day
medical care to hospice in-patients and patients accessing other Specialist
Palliative Care services.

member at the weekly in-patient multidisciplinary team meeting.

hours cover and implementation/ development of the robust out of hours cover.

Cover for colleagues annual leave and
other authorised absences. This post is alongside an in house GP working 2 days
per week.

Educational and clinical supervision of
trainee doctors, advanced clinical practitioners and supervision of Specialty
Doctors. We currently do not have trainee doctors but this will be a future
requirement.

Training and supervision of Nurse
Independent Prescribers.

To participate in education and training
junior doctors, medical students, hospice staff and volunteers.

Participate in clinical audit, quality
improvement projects and clinical governance processes.

Support research activity.

Comply with Continuing Professional
Development (CPD) requirements as stipulated by the Royal Colleges.

To keep a portfolio of evidence, to have
an annual appraisal and write a personal development plan (PDP) in line with
GMC revalidation policies and procedures.

Hospice strategy and objectives.

Work with the leadership team, trustees
and comissioners on the development of Palliative Care and End of Life Care
service.
Proposed
Job Plan
The
initial job plan will be agreed flexibly with the successful candidate and will
be subject to negotiation and review. Three months after commencement, the job
plan will be reviewed and agreed between the appointee and the Director of
Clinical Services. This will be a prospective agreement that sets out the
consultants duties, responsibilities and objectives for the coming year, and
will be reviewed annually. It should cover all aspects of a consultants
professional practice including clinical work, teaching, research, education
and managerial responsibilities. In addition, it should include personal
objectives, including details of their link to wider service objectives, and
details of the support required by the consultant to fulfil the job plan and
the objectives.
On call availability
Out of
hours activity is currently delivered by Derbyshire Health United (DHU)
practitioners who then access on call consultants via South Yorkshire or
Nottinghamshire dependant on area. This will be reviewed and developed in line
with service need. It is possible that an on-call requirement may become
necessary in the future.
Supporting Programmed Activities
The
role will require oversight of clinical education for Palliative and End of
Life Care across Bassetlaw Hospice and to colleagues in partner organisations,
to ensure services are educated and fit for purpose. The appointee is expected
to participate in teaching in collaboration with the education lead.
The
post holder will be encouraged to contribute to research activities at the
hospice and contribute to audit and quality improvement projects.
The
appointee is expected to take part in continuing professional medical
education; study leave is provided and funding is available on application.
Appraisal
Revalidation and Job Planning
Consultants
are expected to undertake the annual Job Planning process which is carried out
in collaboration with the Director of Clinical Services. The Consultant will
bring relevant data on workload in the previous year to the meeting.
There
is a new starter Trust induction programme for all staff and a New Consultant
Induction Programme Prior to commencing post, the Consultant will be informed
of the corporate and local induction processes and programme. It is expected that the Consultant will
participate fully in this and keep up to date with mandatory and statutory
training in line with Trust requirements.
Newly appointed
Consultants will be encouraged to seek a mentor to support their development in
their first year of appointment.
