About the Role

Ashfield North PCN is looking for a compassionate and skilledDementia Specialistto join our team. This unique role is open toeither a Registered Nurse (RMN/RGN/RNLD)or aRegistered Occupational Therapist (HCPC)with advanced experience in dementia care.

You will provide specialistexpertisefor people living with dementia and their families, supporting them from early diagnosis through to endoflife.You will be basedacrossourfour practices with a patient populationofmore than52000 patients.This is a chance to shape how dementia care is delivered across ourfourpractices and make a real difference to families who need thoughtful, skilled support.

Main duties of the job

What You'll Do

  • Carry out complex dementia assessments using nursing or OT frameworks.
  • Support families with distressedbehaviors, crisis prevention, wellbeingplanningand carer strain.
  • Developpersonalisedcare plans and manage complex cases.
  • Lead on dementiarelated clinical advice for GPs and PCN colleagues.
  • Support advance care planning, MCA/Best Interestdiscussionsand endoflife conversations.
  • Deliver dementia training to PCN staff, carehomesand partners.
  • Contribute to dementia pathway development,auditsand quality improvement work.

WhoWereLooking For

You will be a registeredNurseorOccupational Therapistwith:

  • Significant experienceworking with people with dementia and theircarers.
  • Strong assessment,formulationand riskmanagement skills.
  • Confidence working autonomously and within an MDT.
  • Compassion,resilienceand excellent communication skills.
  • A commitment to highquality, personcentereddementia care.
  • Experience delivering training, leading servicedevelopmentor providing psychosocial/functional interventions is desirable.

The post holder will:

  • Deliver specialist, relationshipcentered dementia care.
  • Support people with dementia and their families across the full dementia pathwayfrom early diagnosis to endoflife care.
  • Provide expert assessment, complex case management, and evidencebased interventions.
  • Offer clinical leadership, education, and support to PCN staff and system partners.
  • Contribute to service development, pathways, and dementia strategy work.

KEY RESPONSIBILITIES

Clinical Duties (applies to both professional backgrounds)

  • Provide advanced dementiaspecific assessment usingappropriate professionalframeworks (e.g. Admiral Nurse FrameworkorOT functional assessment models).
  • Manage complex dementia presentations, including distressed behaviours, carer strain, physical and mental health deterioration, and safeguarding concerns.
  • Develop personalised care plans, including risk management, behaviour support strategies, and wellbeing-focused interventions.
  • Act as a key worker for families with complex needs, coordinating care across primary care, mental health, social care, voluntarysectorand urgent care.
  • Support advance care planning, bestinterest decisions, and endoflife conversations.
  • Attend and contribute to MDT meetings and case discussions.
  • Makeappropriate referrals(e.g., MAS/YOD, secondary care, therapies, social care) within professional scope.
  • Provideclinical advice to GPs, ANPs, social prescribers, and other PCN roles.
