Job summary

ABOUT THE ROLE

Whyburn Medical Practice are looking to recruit a full-time Home Visiting Paramedic to join their team. This is a new role that is essential in providing the best quality patient care to patients based in their own home, with on-the-day urgent/acute appointments. With a list of over 12,000 patients, this practice is one of the largest in Nottinghamshire and offers a wide range of primary care services including a home visiting service.

The post holder will be an experience Paramedic who will support and work collaboratively with the Practice Team to carry out home visits that operate during the working day. The successful applicant will have the experience, knowledge and skills to provide holistic care for individuals including assessment, diagnosis, treatment and management.

PICS are supporting our team of Paramedics to develop their skills and knowledge in line with Heath Education Englands First Contact Practitioners & Advanced Practitioners in Primary Care (Paramedic): A Roadmap to Practice.

Reporting clinically to the Lead GP, the postholder will be a valued member of the team and lead the home visiting function to support the work pressures that are evident within GP Practices.

Main duties of the job

KEY RESPONSIBILITIES

  • Work as an autonomous and accountable paramedic

  • Use advanced clinical and communication skills to provide education to patients, promoting self-care and empower them to make informed choices about their treatment

  • Provide clinical sessions for minor illness/injury treatment

KEY REQUIREMENTS

  • Active and relevant professional registration with HCPC

  • Evidence of relevant CPD

  • Extensive experience as a Paramedic

  • Assess and treat patients presenting with minor or acute illnesses in their own place of residence.

  • Conduct urgent on the day appointments/acute appointments as required

  • Triage and treat patients, making any necessary referrals to other members of the primary health care team or organise admission if required.

  • Ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations.

  • Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development, ensuring CPD requirements are met.

  • If it is necessary to expand the role to include additional responsibilities, full training will be given.

Liaison

  • Work closely with the multi-professional, management and administration teams to ensure the smooth running of the GP Practice, reporting any problems encountered to the relevant person and ensuring everyone is aware of the different roles within the team.

  • Work closely with the GP Practice team.

Meetings:

  • It will be necessary to attend and contribute to various PCN and MDT meetings as requested. The only reason for not attending will be annual, study or sick leave or if a non-working day.
