Main duties of the job

KEY REQUIREMENTS

Essential

  • Fully qualified GP with GMC registration and inclusion on the NHS Performers List
  • Completion of GP Specialty Training (or equivalent)
  • Evidence of recent UK general practice experience
  • Strong generalist clinical skills with the ability to manage a broad range of presentations
  • Commitment to good medical practice, clinical governance, and high quality patient care
  • Excellent communication skills and the ability to build rapport quickly with patients and colleagues
  • Ability to work independently and as part of a multidisciplinary team
  • Flexible, adaptable, and comfortable working across multiple sites
  • Evidence of annual appraisal and ongoing CPD
  • Professional integrity, empathy, and a patient centred approach
  • Enhanced DBS check (or willingness for one to be completed)

Desirable

  • Membership of the Royal College of General Practitioners (MRCGP)
  • Additional postgraduate qualifications (e.g., DRCOG, DCH, DFSRH)
  • Competence in joint injections, minor procedures, or womens health skills (e.g., coils)
  • Experience contributing to QOF, clinical audits, or service improvement
  • Experience working across multiple practices, PCNs, or varied clinical settings

JOB PURPOSE

The Portfolio GP will provide highquality, flexible clinical care across our four practices and the Lings Bar service, delivering 10 sessions of planned cover each week as part of a 12month proof of concept model. The postholder will act as an integral member of each practice team on the days they are on site, undertaking the full range of GP duties including clinical consultations ,oncall, task management, medical reports, joint injections(where qualified), QOF related activity, and contribution to enhanced services. Attached to a designated host practice for line management and support, the role is designed to strengthen workforce resilience, reduce reliance on locum provision, and enhance continuity of care. The postholder(s) will be expected to demonstrate adaptability, working flexibly across multiple sites to respond to service needs and maintain safe, effective patient care.

KEY RESPONSIBILITIES

Clinical Care

  • Provide high quality, patient centred clinical care across multiple practices, including face to face, telephone, and online consultations. Conducting home visits, checking and signing repeat prescriptions, dealing with queries, paperwork and correspondence in a timely fashion
  • Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation or externally
  • Assessing the health care needs of patients with undifferentiated and undiagnosed problems
  • Screening patients for disease risk factors and early signs of illness
  • Developing care plans for health in consultation with patients and in line with current practice disease management protocols
  • Providing counselling and health education
  • Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
  • Recording clear and contemporaneous consultation notes to agreed standards
  • Collecting data for audit purposes
  • Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible)
  • Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate
  • Undertake on call duties as required, contributing to safe and effective sameday care.
  • Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate(Lings Bar service)
  • Manage clinical tasks, results, and workflow in line with practice protocols
  • Complete medical reports, insurance forms, and other administrative clinical work
  • Deliver joint injections and other minor procedures within scope of competence
  • Support QOF delivery, clinical audits, and achievement of practice and PCN targets
  • Contribute to enhanced services and local service specifications as required
  • In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care

  • Work flexibly across four practices and the Lings Bar service, adapting to differing systems, teams, and operational needs.

  • Participate in a phased induction to ensure safe and effective integration across all sites.

Teamwork and Collaboration

  • Act as a fully integrated member of each practice team on the days worked, contributing to MDT discussions, clinical meetings, and shared decision making.
  • Build effective working relationships with clinical and nonclinical colleagues across all sites.
  • Engage with the host practice and Medical Director for line management, pastoral support, and professional development.
