Job summary

ABOUT THE ROLE

Employed through PICS but working for Rosewood PCN, we have an exciting opportunity for a RGN to join the PCN to support the delivery of nursing care to patients within the 5 GP practices in Rosewood PCN. The focus of the role is to support Practice Nurse teams with wound care and so the role will suit nurses who have excellent knowledge and experience in wound care but are also enthusiastic to further develop their expertise in this area.

The postholder will provide care in clinics in all 5 PCN practices so will need to be adaptable in their approach and enjoy the variety of working with multiple teams. They will need to be confident to make autonomous clinical decisions but will have support from clinical staff in the practices as well as the ability to refer to specialist tissue viability nurses for their clinical advice and support when needed.

Whilst the majority of care delivery will be wound care, there will be a need to provide a range of other primary nursing care as detailed in the job description. This will provide variety for the post-holder and additional flexibility for the service delivery for PCN patients.

There may be the requirement to occasionally deliver care in the patients home and support PCN pilots and projects as required.

Main duties of the job

KEY RESPONSIBILITIES

  • To provide simple and complex wound care to the PCN population, assessing wounds and developing / implementing/evaluating plans of care.
  • To liaise closely with Practice Nurse colleagues in 5 practices and to seek specialist support from, for example, Tissue Viability Nurses
  • To perform a range of clinical skills such as phlebotomy / ECG / sample collection / vital signs monitoring/diabetic foot checks
  • To support the practice teams with the management of patients long-term conditions
  • Support patients with managing their physical and mental health and well-being through, for example, offering lifestyle advice, promoting immunisation and screening and referring to other health, social and voluntary sector services
  • To administer medications, including immunisations
  • To share expertise in wound care with colleagues and students

KEY RESPONSIBILITIES

Clinical Practice:

  • To assess, design, implement and evaluate safe, creative and effective treatment plans for wound care in collaboration with the patients diverse needs, their carers and other professionals.

  • To take an holistic approach to the assessment of the patients with wounds to include consideration of all the physical, psychological and social factors that affect wound healing.

  • To support the practices with managing patients living with long-term conditions, including frailty, and provide relevant care as necessary.

  • To undertake clinical tasks such as vital signs, ECG, phlebotomy, diabetic foot checks, sample processing

  • Responsible for the safe monitoring, management and administration of medication as required by PICS policy.

  • Work collaboratively with team members and partners to support the PCN GP Practices by contributing to the delivery of immunisations programmes. Collate, collect and monitor data to support immunisation statistical returns.

  • To maintain accurate, timely and relevant clinical records both written and computerised.

  • To be aware of and apply principles on infection prevention and control and to comply with policy, procedure and guidance.

  • To maintain safeguarding responsibilities for children and vulnerable adults in line with PICS policy, and Local Authority procedure and guidance, including making timely referrals as required.

Communication:

  • Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.

  • Communicate effectively with patients and carers, recognising the need for alternative methods of communication to over come different levels of understanding, cultural background and preferred ways of communicating.

  • Develop and maintain excellent channels of communication with Practice Nurse colleagues, GPs and Tissue Viability Specialist nurses regarding the management of complex wounds.

  • Anticipate barriers to communication and take action to improve communication.

  • Estimate and maintain communication with individuals and groups within the PCN and external stakeholders.

  • Act as an advocate whenrepresentingthe patients and colleagues viewpoints to others.

Delivery Quality Service:

  • Recognise and work within own competence and professional code of conduct as regulated by the Nursing and Midwifery Council (NMC 2018).

  • Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.

  • Prioritise, organise and manage own workload in a manner that maintains and promotes quality.
