ABOUT THE ROLE

We are looking for a Senior RGN to join our well-established Enhanced health and care home team in Nottingham West Primary Care Network. The team supports the delivery of the Enhanced Health and Care Home element of the PCN Direct Enhanced Service (DES) and supports both GP practices and Care Homes within Nottingham West PCN to provide holistic care to Care home residents.

The post holder will liaise with the Care Coordinator team to ensure that a full holistic review for all newly admitted residents and recently discharged from hospital residents are in place and that annual reviews of the care plans occur. The post holder will work with the Community Geriatricians to ensure the delivery of the Monthly MDT.

The post holder will work as a clinical care home lead in the team conducting ward rounds and completing actions as required including the creation of Care plans and Respect form and supporting practices with Long term condition reviews, falls assessments etc.

Main duties of the job

  • To establish and improve professional standards of care in the management of residents within care homes by developing partnerships with key stakeholders.

  • Utilising and interpreting data, to support service improvement for care homes and primary care, as well as understanding the demands on urgent and emergency services to reduce avoidable activity where possible. Data and local intelligence will be utilised to develop and influence new ways of working to improve patient care, the resilience of primary care and system partners.
  • The post holder will be required to evidence the impacts of the post and provide data monitoring to further shape the role and delivery of the Care Home DES within the PCN.
  • As a senior clinician the post holder may be required to support local and system wide meetings as appropriate and have a visible presence across the care homes, GP practices and Integrated Care Board work streams which have relevance for care homes.
  • The post holder is responsible for ensuring that the residents and care home managers within NW PCN locality receive assistance in accessing health care interventions by signposting to the most appropriate person to provide care.
