ABOUT THE ROLE
We have an exciting opportunity for Nursing Associates to join our dynamic frailty team in Sherwood Primary Care Network.
Sherwood PCN includes the following GP Practices:
- Abby Medical Group
- Bilsthorpe Surgery
- Hillview & Farnsfield Surgery
- Major Oak Medical Practice
- Middleton Lodge Practice
- Rainworth Primary Care Centre
- Sherwood Medical Partnership
We are looking for a Nurse Associate who is enthusiastic about providing high quality care in primary care. The role will involve closely working with an Enhanced Practice Nurse, delivering care to housebound patients in their own homes. It will include contributing to the annual frailty review - an holistic assessment of the impact of aging and illness on their life including assessing the risks associated with frailty such as falls, malnutrition and skin integrity. The postholder will undertake clinical tasks such as immunisation, ECGs and phlebotomy. The postholder will require a degree of flexibility to fit the needs of the service.

KEY REQUIREMENTS
Nurse Associate registered with NMC
Nurse Associate Foundation Degree
GCSE Grade A-C in Maths & English or skills level 2 Maths & English or equivalent
