ABOUT THE ROLE
Employed through PICS but working for Mansfield North, we have an exciting opportunity for an RGN (fixed term contract to cover maternity leave) to join the PCN to support the practices in the delivery of Primary Care nursing to housebound patients. The role will be based in the community, delivering nursing care on behalf of the GP practices to patients in their own home including care homes. There may be the requirement to occasionally deliver care within the GP practices and support PCN pilots and projects as required.
We are looking for an RGN who is enthusiastic about providing high quality care. The successful candidate will be able to undertake annual reviews for long-term conditions, adult immunisations and other clinical procedures. They will need to have a good knowledge of the Quality and Outcome Framework and frailty. A non-medical prescribing qualification would be advantageous but not essential.
The postholder will need to be quick to learn, hardworking, highly motivated and have the confidence to work independently as well as part of a team. They will need to be able to support junior colleagues in the team.
The postholder will be supported by the existing Community Practice Nurse and the PICS Lead for PCN nursing.

Main duties of the job
KEY RESPONSIBILITIES
To undertake annual reviews of housebound patients
To undertake frailty reviews, immunisation programmes and primary care clinical procedures
To work closely with nursing and administrative colleagues in the team
To liaise with GP practice colleagues to ensure seamless care between the community team and GP practice
To refer patients to other health, social care and voluntary services as required
KEY REQUIREMENTS
NMC registration as a Registered General Nurse
Post registration long term conditions education
Primary or community nursing experience
