ABOUT THE ROLE

We have an exciting opportunity for a Senior Clinical Pharmacist to join and lead our dynamic team. We are looking for someone who is enthusiastic, self-motivated, well organised, and passionate about providing high quality care - preferably with previous experience of working in primary care, to develop and manage medicines optimisation services within our practices.

You will be based within Ashfield North PCN with a patient population in excess of 52,000. You will be part of a wider team of Clinical Pharmacists and Pharmacy Technicians and supported by our PICS leadership team. You will act as a mentor to help develop our current team of Clinical Pharmacists and Pharmacy Technicians and will work on PCN contracts as well as supporting the practices with their general prescription/medication work. You will also spend a portion of time doing the role of a Clinical Pharmacist depending on the number of hours you work each week.

Main duties of the job

KEY RESPONSIBILITIES

  • To develop and support the Clinical Pharmacist Team (9 Clinical Pharmacists and 3 Pharmacy Technicians). To line manage staff and conduct annual appraisals etc
  • Provide clinical leadership on medicines optimisation and quality improvement, and manage some aspects of the quality and outcomes framework and enhanced services
  • To help plan, develop and support the introduction of new working processes within the PCN to optimise the quality of prescribing
  • To audit and assess the prescribing quantity, frequency, shared care protocols in records, and blood tests areas to audit;
  • DMARDs
  • NOACs
  • Controlled drug and drugs causing dependency
  • NSAIDs
  • Lithium
  • Sodium valporate in females at risk of pregnancy
  • Inhalers
  • Antibiotic prescribing
  • Atrial fibrillation CHADVASC, HASBLED, CVA risk reduction
  • Nationally identified low priority medicines
  • Liaise with the clinical pharmacist lead for PICS and actively participate in regular Senior Clinical Pharmacist meetings (2 monthly)
  • To work at each of the 4 PCN practices located around Sutton In Ashfield

KEY REQUIREMENTS

  • Minimum of 3 years experience as a pharmacist, preferably demonstrated within a practice portfolio
  • Have experience and an awareness of common acute and long term conditions that are likely to be seen in general practice
  • Preferably completed/to get enrolled on CPPE Pathway for Clinical Pharmacists
  • May hold/be working towards an independent prescribing qualification
  • Be working at/willing to work towards, Advanced Clinical Practitioner Level
