Job summary

The team at Lings Bar is made up of GPs, ANPs, Pharmacist and Care Navigator. PICS support primary care cover into Lings Bar. Lings Bar has 72 rehabilitation beds and is open Monday Friday from 8am to 6.30pm (excluding Bank Holidays).

The post-holder will manage a caseload of up to 72 and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care.

The sessions will be worked on Thursdays and Fridays between 10:00-16:30 on site.

Main duties of the job

KEY RESPONSIBILITIES

  • The post-holder will be available on site to review patients who are transferred to the unit.
  • Providing General Practice level care for residents.
  • Leading MDTs which are scheduled on a weekly basis.
  • Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
  • Assessing the health care needs of patients with undifferentiated and undiagnosed problems

KEY REQUIREMENTS

  • GP Specialist training
  • GMC registration
  • Experience as a GP

Job responsibilities

JOB PURPOSE

The post-holder will manage a caseload of up to 72 patients who have been classed as medically stable and are admitted to Lings Bar for rehabilitation unit in Gamston, Nottinghamshire prior to discharge (home first). Patients remain temporary residents with PICS and are temporary registered to its Hama Surgery. The Length of Stay LOS at Lings Bar is expected to be approx. 21 28 days when patients will be discharged back to their own practice or supported to register with a new practice is moving to a new address post discharge.

The team at Lings Bar is made up of GPs, ANPs Pharmacist, Care Navigator and senior operational oversight. PICS provide primary care cover into Lings Bar on Monday Friday from 8am to 6.30pm (excluding Bank Holidays).

The post holder will need to deal with a wide range of primary care health needs in a rehabilitation unit working closely with Nottinghamshire HealthCare Trust staff who provide 24/7 nursing and rehabilitation staff along with hotel and facilities management, PICS expect the delivery of the highest standards of care for these temporary patients.

KEY RESPONSIBILITIES

Clinical:

  • The post-holder will be available on site to review patients who are transferred to the unit.
  • Providing General Practice level care for residents.
  • Leading MDTs which are scheduled on a weekly basis.
  • Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within Lings Bar.
  • Assessing the health care needs of patients with undifferentiated and undiagnosed problems
  • Screening patients for disease risk factors and early signs of illness
  • Developing care plans for health in consultation with patients and in line with current practice disease management protocols
  • Providing counselling and health education
  • Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
  • Recording clear and contemporaneous consultation notes to agreed standards
  • Collecting data for audit purposes
  • Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible)
  • Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate
  • In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
