Primary
Integrated Community Services is seeking a dynamic and dedicated Diabetes
Specialist Nurse to join our team within the PICs Community Services working
with Nottingham West PCN. The successful candidate will work within community and clinic
settings, collaborating closely with the practice nurse team to deliver expert diabetes
advice, care, and support to patients. This role will involve being a clinical
specialist in diabetes assessment, intervention, and support, as well as
providing specialist nursing care while maintaining and enhancing service
quality. The post holder will play a vital role in facilitating new ways of working
across professional boundaries, supporting primary care staff with specialist
training, and participating in structured educational programs for patients.
Main duties of the job
KEY
RESPONSIBILITIES
- Conduct diabetes assessments,
interventions, and support within community clinics, ensuring the delivery
of high-quality care.
- Provide specialist nursing care
to patients, continually developing and maintaining service standards.
- Facilitate collaboration between
primary care and other healthcare organizations, acting as a link between
secondary care specialist services and community services.
KEY
REQUIREMENTS
- Qualified and NMC registered nurse
with V300 NMP qualification.
- Clinical expertise: Proven
experience as a clinical specialist in diabetes assessment, intervention,
and support within community settings.
- Nursing proficiency: Registered
Nurse with extensive experience in providing specialist nursing care,
preferably in diabetes management.
- Collaborative skills: Ability to
work across professional boundaries, facilitate training for primary care
staff, and support structured educational programs for patients.
- Travel between sites using own
car with full UK driving licence with business insurance

The post holder will be employed by Primary Integrated Community Services, with sessions in a community and clinic setting within Nottingham West PCN, alongside the practice nurse team. This will include delivery of expert diabetes advice, care and support to patients, primary care and other healthcare organisations professionals. The post holder will be expected:
- To be a clinical specialist in Diabetes assessment, intervention and support within Community clinics
- To provide specialist nursing care, maintaining and developing a quality service.
- To facilitate new ways of working across professional boundaries.
- Use knowledge of medicines and prescribing skills to influence the reduction of risk and complications associated with medication and polypharmacy.
- To provide specialist training to primary care staff within the community.
- To support the structured educational programme for patients in primary care and communities
- To become a link between secondary care specialist services and community services
KEY JOB RESPONSIBILITIES
Clinical:
- Work autonomously to set up and deliver a specialist service, based on principles that are evidence based and which enhance the quality of patient care.
- Support GP services to offer insulin/Diabetes medication initiation to their registered population within the patients own practice, via face to face, email and telephone support.
- Support GP services to offer insulin initiation and disease management to their registered population within the patients own practice. Undertake domiciliary visits as required.
- Assess, diagnose, discharge and refer patients / clients within a defined area of competence in partnership with the patients registered practice.
- Be a visible point of contact for patients, visitors, relatives, and staff, acting as a resource for problems and able to clearly present the patients / clients point of view to others. Receive and investigate any complaints received.
- Ensure that patient care plans follow clinic / service guidelines are regularly updated, and changes implemented ensuring that nursing records and documentation are maintained and current.
- Liaise with senior clinicians in primary care and NUH to arrange meetings to discuss patient management as required.
- Participate in the development of evidence based patient care policies and procedures. Develop and contribute to clinical guidelines and protocols and lead the implementation process as required.
- Provide specialist support to clinical teams in dealing with the complex needs of different client groups and when imparting or receiving sensitive, complex or contentious information.
- Review and develop services to support management of Diabetic patients in Nursing Homes
- Participate in the identification and facilitation of clinical audits to promote clinical effectiveness

- Liaise closely with other health and social care professionals to ensure a coordinated approach and high quality care is provided for patients.
- Convey and present specialist information, written or verbal, formally or informally, as appropriate. Report appropriate clinical information to senior staff. Ensure that written documentation is kept in respect of all investigations and incidents.
- Establish professional relationships and maintain effective communication.
- Link with professional networks across the PCN/ICB and wider health community, in order to develop and exchange ideas for improving clinical care.
- Convey complex and sensitive information (eg. abnormal test results) in a comprehensible and empathic manner.
- Discuss all alternative treatment options and facilitate clinical management decisions.
- Obtain informed consent for procedures as required, in accordance with the consent policy.
- Provide ongoing liaison and support for patients / clients across functional boundaries.
