Job summary
We have an exciting opportunity for an Advanced Nurse Practitioner or Advanced Clinical Practitioner to join our dynamic Care Homes team. We are looking for someone who is well organised and enthusiastic about providing high quality care to patients. The role will mainly involve leading our weekly care home rounds by acting as an autonomous practitioner working independently and in conjunction with other health care professionals to assess, diagnose and treat the conditions of patients residing in the PCN care homes, to provide holistic patient care.
You will be based in the community, visiting care home residents registered with practices in Newark PCN and there will be some remote working. Newark PCN is made up of 7 GP Practices

Newark PCN has a patient population in excess of 78,000. You will be part of our PCN Care Homes Team and supported by our Clinical Directors and PCN Business Manager. The Newark PCN Care Homes Team includes an Advanced Nurse Practitioner, Dietician, Clinical Pharmacist, three Nursing Associates/Trainees (time split between Care Homes and Practices) and a Care Coordinator.
Main duties of the job
KEY RESPONSIBILITIES
- Lead weekly care home rounds with the support of the PCN Care Homes team.
- Undertake clinical practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to residents of PCN care homes.
- Assess, diagnose and treat residents with a range of acute, non-acute and chronic medical conditions.
- Attend weekly Care Home Team meetings and attend PCN meetings as necessary.
KEY REQUIREMENTS
- Registered General Nurse or Paramedic (Independent Non-Medical Prescriber desirable).
- Minimum of 5 years post registration experience as an RN/Paramedic.
- Ability to assess and manage patient risk effectively and safely.

- Assesses diagnoses and treats residents with a range of acute, non-acute and chronic medical conditions.
- Undertake telephone consultations.
- Refers patients to other medical specialities for assessment if deemed appropriate.
- Refers patients as appropriate to other members of the multidisciplinary team, secondary care and to other statutory and voluntary organisations.
- Interprets a range of diagnostic tests and routine clinical procedures.
- Makes critical judgements of the highest order to satisfy the expectations and demands of the job eg first contact with patients presenting acutely and manages care in the presence and absence of protocols, including prescribing or deprescribing as appropriate to the clinical situation.
- Makes decisions where precedents do not exist, where appropriate without recourse to others e.g. direct referral of patients to hospital specialists without prior reference to other medical colleagues.

CARE AND PROGRAMME MANAGEMENT
Manages the Enhanced Health in Care Homes programme for designated care homes which includes:
- Undertaking weekly home rounds
- Contribute to person centred holistic assessments of all patients admitted / readmitted to a care home within 7 days
- Contribute to the development of personalised support care plan for residents within 7 days of re/admission
- Develops and manages highly specialised programmes of care and care packages and provides highly specialised advice concerning care normally beyond the scope of normal nursing practice e.g. establishes differential diagnoses, management/treatment plans and makes referrals as appropriate.
- Works with care home colleagues to identify residents who may be approaching end of life and puts into place appropriate plans which may include for example, prescribing anticipatory medicines, making fast track referrals and discussing escalation of care/DNACPR with families and carers.
- Communicates highly sensitive condition related information to patients, relatives and carers.
- Initiates the process of diagnosis with patients suspected to have a chronic disease e.g. diabetes, COPD, asthma, IHD, referring to other clinical staff as appropriate.
- Prescribe safely and appropriately in accordance with non-medical prescribing policy and protocols
- Where appropriate, use prescribing competencies to initiate medicine
management reviews and prescribe medicines and appliances for patients via
independent prescribing arrangements. Utilise advanced communication
skills to facilitate the understanding and concordance of medicines with
patients and carers.
- Contribute to the PCN practices targets both local and national within the disease management agenda eg QOF, dementia reviews
- Contributes to disease registers.
- Audits outcomes of care against standards and initiate changes as necessary.
- Refers to other members of the extended primary health care team as appropriate eg clinical pharmacist, dietician, specialist nurses.
- To actively implement safeguarding protocols for vulnerable adults and to know who to refer to and how to escalate concerns about radicalisation.
