Job summary
ABOUT THE ROLE
Sherwood PCN is seeking an experienced Dietitian to provide dietetic support to care home residents and training to staff. You will be working closely with the care home multi-disciplinary teams and community teams. You will also have the opportunity to work collaboratively with other PCN services to ensure each patient is treated holistically. For example, linking in with our PCN Health and Wellbeing Coaches who are working with patients to manage long-term conditions, ensuring that positive behaviours are established and maintained.
PICS are supporting our team of Dietitians to develop their skills and knowledge in line with Heath Education Englands First Contact Practitioners & Advanced Practitioners in Primary Care (Dietetic): A Roadmap to Practice.
You will be part of a wider team of PICS Dietitians and supported by our Dietitian Clinical Lead who is on the PICS management team.
Main duties of the job
KEY RESPONSIBILITIES
- Provide an enhanced dietetic service to frail patients, specifically those in care homes or those with complex chronic health conditions in the community
- Make dietetic advice readily available
- Develop and deliver nutrition teaching programmes to care homes
KEY REQUIREMENTS
- Bachelor of Science degree in Nutrition & Dietetics or equivalent
- Registered with the Health and Care Professions Council (HCPC)
- Evidence of continuing professional development and willingness to undertake the FCP Roadmap (Dietetics)
