Job summary

We are looking to recruit a Social Prescribing Manager to lead and develop the Social Prescribing Link Worker Service across Primary Care Networks (PCNs) in Nottingham North and East, and Mid Nottinghamshire.

As the Social Prescribing Manager, you will be a positive advocate and spokesperson for the benefits of social prescribing and the personalised care agenda on behalf of PICS and the PCNs. The core duty of your role will be to provide daily operational and strategic management through excellent leadership and support for the teams under your management.

It is crucial you work collaboratively with key stakeholders to engage partners, ensure strong communication, minimise risk and promote the success of the service delivery through a variety of mechanisms.

Main duties of the job

KEY RESPONSIBILITIES

  • To provide professional leadership of the PCN Social Prescribing Teams
  • To strategically develop service improvements
  • To manage people and your own workload
  • Produce service reports and data referring to KPIs and service contract specifications. Identify trends, issues and suggest improvements or solutions.

  • Experience of line management responsibilities of other members of staff/teams
  • Evidence of Continued Professional Development
  • Ability to manage own workload and that of others to achieve KPIs

JOB PURPOSE

  • Work collaboratively with key stakeholders internally and externally to engage partners, ensure strong communication, minimise risks and promote the success of the service delivery through variety of mechanisms.
  • Be responsible for managing day-to-day planning, implementation and problem-solving approach to ensure their team achieves their goals.
  • Address staff scheduling, monitoring the teams performance though structured supervision, to ensure they meet their goals, verifying individual performance by using tracking systems; coaching team members to improve performance, motivating team members, leading and attending meetings and training programs.
  • To review and challenge current service operation to improve and streamline services.
  • Oversee the patient experience using patient feedback and forums to evaluate the service.
  • Work closely with Management and Clinical Teams to ensure a mutual understanding of needs and requirements for the service.
  • Work in partnership with the Community Voluntary Services around community development proposals ensuring good working relationships with CVS partners and development of their Social Prescribing offers in line with the CVS Alliance.
  • Ensure representation at key meetings in relation to health & wellbeing, health inequalities and the wider determinants of health.
  • Ensure that the team is managed in a consistent, fair and equitable manner, dealing with issues appropriately and with reference to PICS Policies and Procedures.
  • Oversee the welfare of the team, taking advice and seeking support from Senior Management or HR as appropriate.
  • Investigate and resolve complaints using PICS policies, procedures and protocols.
  • Planning future needs of the service, including budget reports and recruitment, training and development of staff to meet future needs.
  • Produce service reports and data referring to KPIs and service specifications. Identify trends, issues and suggest improvements or solutions.
  • Ensure Social Prescribing KPI targets are met for the service.
  • Work with PICS data team to ensure deadlines are met to submit quarterly data reports, highlight reports and case studies to NHS England and Commissioners.
  • Attend quarterly data meetings with NHS England and Commissioners to present the teams data/KPIs, along with highlighting service delivery plans, risk and challenges to service delivery.
  • Work closely with the Head of Social Prescribing, Commissioners and the Place based Partnership representatives to develop the service and contracts responsible for.
  • Work closely with the ICS to develop the Social Prescribing service in line with ICS priorities.
  • Have a flexible and supportive approach to requests for information about the Social Prescribing service from stakeholders.
  • Recruitment, induction, on-going supervision/appraisal and training & development of staff.
  • To deal with problems and issues that may arise within the team, always modelling and enforcing professional standards of behaviour.
