Job summary

Do you have the personal qualities and experience to be able to work within a multi-disciplinary team to provide coordination and administrative support to the clinical team and other members of the PCN? We are looking for an individual who can work with the housebound nursing team, care homes, general practice and existing services to support the coordination to patients in their own homes or in a residential home. Part of this includes identifying Housebound/Care Home residents who require an annual health check review under the QOF (Quality and Outcomes Framework) system.

You will be based within Rosewood PCN, covering:

  • Acorn Medical Practice, 11-13 Wood Street, Mansfield, NG18 1QA
  • Churchside Medical Practice, Wood Street, Mansfield, NG18 1QB
  • Forest Medical Group, Oak Tree Lane Surgery, Jubilee Way South, Mansfield, NG18 3SF
  • Forest Medical Group, Rosemary Street Health Centre, Rosemary St, Mansfield, NG19 6AB
  • Millview Surgery, 1a Goldsmith Street, Mansfield, NG18 5PF
  • Roundwood Surgery, Wood Street, Mansfield, NG18 1QQ

Rosewood PCN has a patient population in excess of 49,900. You will be part of a wider team of Care Coordinators and supported by our Care Navigation Manager who is on the PICS leadership team.

Main duties of the job

KEY RESPONSIBILITIES

  • Produce, maintain, and monitor the practice appointment system for the PCN Housebound Nursing team.
  • Carry out system searches to identify Housebound QOF patients.
  • Support the Nursing Team, PCN staff and Practice manager with administrative work including contacting patients and booking appointments through an appointment system.

KEY REQUIREMENTS

  • Good standard of education GCSE or equivalent in English and Mathematics
  • Experience of office procedures working at a high level as part of an administration team / within an administration role
  • Understanding and able to deal with confidential and sensitive issues when liaising with team members / other professionals

JOB SUMMARY

The post holder will work within the PCN Team, providing coordination, administrative and support to the Clinical teams and other members of the PCN.

The post holder will work closely with Housebound Patients, Care Homes, General Practices, and existing services by providing administrative support, coordinating, and organising clinical rotas for the housebound nursing team, making sure to follow the Quality and Outcome Framework system (QOF) by helping the PCN provide good quality care to patients in their own homes or in a residential home.

The post holder will support the PCN to proactively identify Housebound/Care home patients who require an annual health check review under the QOF (Quality and Outcomes Framework) system.

ROLE OBJECTIVES:

The post holder, working closely with the PCN team, Care Coordinators, PCN Housebound Nursing Team, Learning Disability Nurse, GPs and existing community services will:

  • Produce, maintain, and monitor the practice appointment system for the PCN Housebound Nursing team.
  • Carry out system searches to identify Housebound QOF patients.
  • Support the Nursing Team, PCN staff and Practice manager with administrative work including contacting patients and booking appointments through an appointment system.

ADDITIONAL RESPONSBILITIES:

  • Produce, maintain, and monitor clinical rotas for PCN Housebound Nursing Team on a clinical system
  • Managing Housebound group task for the PCN.
  • Liaise with patient and arrange routine appointments in an appropriate time frame, prioritising urgent appointments.
  • Run clinical searches to identify Housebound/Care home patient who have a QOF indicator.
  • Run reports to identify Learning Disability patients needing an annual health check.
  • Identify Housebound QOF patient based on patient birthday month.
  • Administrative support to all PCN housebound nursing staff including Learning Disability Nurse.
  • Action emails from other teams/services that may need support from the PCN Housebound Nursing team.
  • Support PCN staff and Practice Managers with all Housebound patient queries.
  • Arrange emergency cover from the PCN Housebound Nursing team for PCN Practice Managers for when there are staff shortages.
  • Attend regular PCN Nursing MDT meetings.
  • Communicate effectively with other PCN staff who may need support from the PCN Housebound Nursing Team/Learning Disability Nurse.
  • To contribute to the integration of Health and Social Care by maintaining up to date recording systems for all agencies within the PCN Team and providing information to any member of the PCN Team in order to ease processes and communication in agreement with Data Protection protocol
  • To be responsible for recording, reporting and producing evaluation reports which will include evaluation detailing effectiveness outcomes of new roles.
  • To be customer (patient, carer, GP) focused when representing the service
  • To work collaboratively with other teams and services to maintain an effective and efficient service
  • To offer appropriate support and guidance to patients and their families/carers
  • To plan / organise work using own initiative, whilst being able to work as a valuable member of a team

  • To work effectively as part of a team to provide cover for Care Coordination Teams when required and to be flexible regarding working hours to meet the needs of the service

  • To use a range of verbal and non-verbal communication tools to communicate effectively with patients, carers and families and colleague
  • Provide coordination of and participate in relevant internal and external working groups and provide project advice, expertise and support were requested
