Job summary
Are you an experienced Practice Nurse, specialising in Chronic Disease e.g: respiratory / COPD / Asthma? Looking for a new challenge in Primary Care?
Symphony Healthcare Services (SHS) has an exciting opportunity to join forward-thinking practices based in Martock and South Petherton in Somerset.
This is a permanent, part time post of 30 hours per week.
The annual salary range for this post is £39,146.36 - £41,851.01 per annum (pro rata for part time hours, depending on experience).
Main duties of the job
The main duties of a SHS Chronic Disease Practice Nurse are:
- Manage a designated case load of patients with chronic conditions, visiting them in their own home or consulting with them at the practice.
- Work within the NMC Code
- To mentor junior staff ensuring that they are supported and continually developed.
- To develop the nursing services offered both for the practice and through SHS.
- To support clinical Supervision within the practice.
- To work with the practice and Lead Nurse in the development of clinical skill mix
- Promote SHS values and acts as a role model.
- To Implement the SHS Nursing Strategy as defined by the Nursing and AHP Director and the Lead Nurses forum.
- To ensure a high level of patient involvement and satisfaction with the service, including linking in with the practice PPG.

Symphony Healthcare Services (SHS) was established in 2016 as part of the award-winning Symphony Programme Vanguard, developing new models of care. We now support 16 practices across Somerset providing care for over 100,000 patients.
Symphony has developed a model where each practice is encouraged to operate autonomously, but as a subsidiary of NHS Somerset Foundation Trust (SFT), and with the backing and economies of scale from a central support team (particularly for HR, finance, corporate and strategic management matters).
