Job summary

Symphony Healthcare Services are looking for a highly organised and accurate Medical Records Summarisers / Administrators to support our clinical and administration teams by ensuring patient records are clearly summarised, well-coded, and safely maintained.

You will begin by supporting multiple practice sites, including Buttercross health centre in Somerton, Hamdon Medical Centre in Stoke-sub-Hamdon and South Petherton Medical Centre(to learn local processes and help standardise record quality), then transition to being fully based at Crewkerne Health Centre, where the role will focus on ongoing summarising work, document workflow, and core administrative support.

We have both full and part time permanent hours available - please state your preference in your application.

The rate of pay is £12.65 - £12.95 per hour and you have the option to join the NHS pension, plus access to many other employee benefits.

Main duties of the job

  • To review medical records and produce an accurate summary of the patients medical history, within the Practices protocols and agreed timescales.
  • Enter Read Code Information onto the Practices computer system.
  • Use medical knowledge to correct mistakes found in existing patient records.
  • Adhere to and monitor progress of notes summarising against Practice targets, maintaining structure and protocols when recording new information.
  • Be aware of specific Read Codes and associated records, following Practice protocols for recording, obtaining, and filing patient records as appropriate e.g., child protection.
  • Assist GPs and other Practice staff with queries relating to medical records.
  • To participate in audits of data collection standards in the Practice.
