Job summary

We have an exciting opportunity to join our Personalised Care group within a dynamic and innovative Primary Care Network.

We are looking for a highly motivated and dedicated Care Coordinator to join our friendly and supportive practice for up to 25 hours a week.

£12.50 per hour

Main duties of the job

We have an established Personalised Care group within our PCN, consisting of Care Coordinators, Health & Wellbeing Coaches and Social Prescribing Link Workers, who work collaboratively to provide non-clinical support.

As a Care Coordinator, you will play an important role within a PCN to proactively identify and work with people, including the frail/elderly and those with long-term conditions, to provide coordination and navigation of care and support across health and care services.

Your role will ensure patient health and care planning is timely, efficient, and patient-centred. You will be responsible for the coordination of the patients journey through primary care and secondary care.

You will work for the PCN but will operate from one of the practices.

Job responsibilities

The post holder will operate as part of the PCN Personalised Care Group but will be based in one of the PCN practices.

Care coordinators play an important role within a PCN to proactively identify and work with people, including the frail/elderly and those with long-term conditions, to provide coordination and navigation of care and support across health and care services.

They work closely with GPs and practice teams to manage a caseload of patients, acting as a central point of contact to ensure appropriate support is made available to them and their carers; supporting them to understand and manage their condition and ensuring their changing needs are addressed.

This is achieved by bringing together all the information about a persons identified care and support needs and exploring options to meet these within a single personalised care and support plan, based on what matters to the person.

Care coordinators, review patients needs and help them access the services and support they require to understand and manage their own health and wellbeing, referring to social prescribing link workers, health and wellbeing coaches, and other professionals where appropriate.

Essential

  • Experience of administrative duties Working in a multi-disciplinary setting where influence and negotiation is required Knowledge/familiarity with medical terminology

Desirable

  • Experience of working with healthcare professionals and or previous experience in the NHS or social care or relevant field. Understanding of current issues facing the NHS Understanding of health and social care processes

  • GCSEs/Diploma/ HNC level (or relevant experience)
