Job summary

We have an exciting opportunity to join our growing Personalised Care group within a dynamic and innovative Primary Care Network.

We are looking for highly motivated and dedicated Health and Wellbeing Coach to bolster those we already have in our practices.

The Primary Care Network includes practices in Martock and South Petherton, Stoke Sub Hamdon, Crewkerne and Somerton, although the position we are seeking will operate mostly from Martock and South Petherton.

Hours: 30 per week

Salary: Between £11.70 and £12.50 per hour

Main duties of the job

We have an established Personalised Care group within our PCN, consisting of Care Coordinators, Health & Wellbeing Coaches and Social Prescribing Link Workers, who work collaboratively to provide non-clinical support.

As a Health and wellbeing coach, you will predominately use health coaching skills to support people to develop the knowledge, skills, and confidence to become active participants in their care so that they can reach their own health and wellbeing goals.

You may also be required to provide access to self-management education and peer support. Our Health and Wellbeing coaches support people to allow them to self-identify existing issues and encourage proactive prevention of new and existing illnesses. This approach is based on using strong communication and negotiation skills and supports personal choice and positive risk taking.

You will work alongside people to coach and motivate them through multiple sessions, supporting them to identify their needs, set goals, and help them to implement their personalised health and care plan.
